Bulbs can be a bit funny. If you look at the element it may look perfect but as it warms up it could be breaking contact. You say you've changed the buld holder but you haven't said you replaced the bulb with a new one, change the bulb for what they cost then you know its good?

I know this is an old post but figured I'd comment in hopes of helping others.
I also have spent a lot of time trying to figure out this same problem. Finally solved it yesterday. Turned out being the body control module.

That is very vague body control model (BCM).
